Ability to Change Care Plan without changing scheduled Review Date

There appears to be no way to make small changes to the care plan without changing the formal review date. We are regularly asked by public health nurses to change something small on the care plan but that does not come with a full review of the rest of the care plan. With the new set up it appears we cannot make a change like that without messing up the review date.

Simo Hännikkälä 1. A PHN may decide a client needs to go for a walk or reduce/increase meds or change their shower days. There are a million reasons why we might do that but not do a full review of careplan. 2. Happens very regularly. I would think at least 1 care plan a week would or at least should be updated like this. 3. The review date determines when staff are required to do a review and they need to review all care plans at least every 6 months. If they make a change and don't do a review but the system tells them they did a review, we can no longer report on the frequency of reviews accurately.
